The American Immunization Registry Association (AIRA) has developed a resource titled, Analytic Guide for Assessing Vaccination Coverage Using an IIS. The purpose of this project was to develop a framework for helping those conducting IIS population-based immunization coverage assessments, with a guide as the output.

AIRA helps develop resources for use amongst IIS and immunization programs (primarily U.S. based) and relies heavily on its members - the IIS and immunization program staff. For this project, with input from members and AIRA staff, a consultant convened a small workgroup composed of subject matter experts, primarily epidemiologists, with extensive experience using IIS data for research and coverage assessments. The consultant and the workgroup gathered and reviewed materials and resources, including real experiences in using IIS data.

A series of key decision points was identified to assist with the design of an assessment. Starting with the purpose of the assessment which drives many of the decisions made in defining the assessment criteria. Two of the most common purposes are to determine the protection level of the population, or to determine the percentage of the population that has been vaccinated. After determining the purpose of the assessment, other key decisions must be made including:

  1. Determine the cohort, who will be excluded from the analysis
  2. Vaccination-related criteria such as which vaccines will be assessed
  3. Selecting the denominator, IIS-based, non-IIS-based (e.g., census, school, etc.)
  4. Data Quality, if not addressed, it can significantly affect the assessment including the a. deduplication process, b. the evaluation/forcasting/clinical decision support for immunization, c. IIS/EIR maturity and completness

Key DEcision Points

In addition to defining key decision points, examples of real coverage assessments have been documented in an addendum (soon to be released), including the key decisions for each assessment and the logic for their key decisions.

This work provides a foundation for using an IIS for population-based immunization coverage assessments including practical considerations and approaches that may be tailored to the needs of an assessment and to the functionality of each IIS.

While the data source of this project was an IIS, many of the methods can be adapted for use with other data sources.
