POST 00710E : LOGISTICS TRAINING Follow-up on Posts 00689E, 00697E, 00703E and 00707E 25 August 2004 ___________________________ Auguste Ambendet (mailto:[email protected]) from WHOAFRO responds to Jean Bomani while drifting away somehow from the topic of training as such. ___________________________ Dear all, I would like to respond to Jean Bomani from EPI/Tanzania in relation to supervision of immunization services. The District Health System is an integrated health system. The Health District constitutes a territorial entity made of a network of Health Centres or peripheral Health Units, which gravitate around a referral hospital and is under the management of a District Management Team. Because of the lack of personnel in rural settings and considering the peculiar relationship between the health system and the population, peripheral health unit staff is polyvalent. It is the same personnel which give curative and preventive care (including immunization), integrated care (curative, preventive and promotional), and continuous care (referral to District Hospital and counter-referral). Generally, hospitals do have personnel in charge of logistics. If the hospital is well rationalized, the logistics agent is integrated to the district management team. As far as the District Management Team is concerned, the modalities have for a long time been forgotten. But with the launching of the "RED" approach (Reach Every District) or ACD (Atteindre Chaque District) in French, some changes are being introduced. The case of the Health District of Bafia in Cameroun is a good example. The implementation of the RED approach started with strengthening of the district organizational structure, whereby the following units were put into place: EPI Unit in charge of vaccines and Cold Chain equipment, Information and Statistics Management Unit for EPI, and Social Mobilisation Unit. In conclusion, everything is centered round the district health system and it is therefore important to take into consideration the main characteristics of this system, i.e. an integrated continuous health care, polyvalent personnel and a decentralized system.
