Approximately 1.9 billion people live in fragile and conflict-affected settings, yet vaccine design and evaluation continue to assume stable delivery conditions.
This chapter challenges that paradigm.
-Drawing on evidence from vaccine rollouts - including nOPV2, measles-containing vaccines, Mpox, and Ebola vaccines - in the Democratic Republic of the Congo, Liberia, South Sudan, Nigeria, the chapter introduces the Field-Anchored Vaccine Specification Matrix (FAVSM): a framework that translates frontline operational realities - cold chain failures, population displacement, insecurity, workforce constraints - into concrete product design requirements.

This is a topic I have thought about deeply from years of field epidemiology, outbreak response and immunization work across Africa and beyond, and I am glad to finally contribute to the scholarly conversation on it.
