Hi, TechNet Friends.
We are conducting a short research survey on vaccine cold chain reliability and would welcome your participation. The survey is part of a study applying a behavioral science framework to the operational practices that sustain immunization supply chains — an area where the experience of frontline practitioners like you is directly relevant.
This survey is for people working at the facility or subnational (provincial/district) operational level who are currently involved in vaccine cold chain management, equipment maintenance, immunization logistics, temperature monitoring, or cold chain supervision.
The survey takes approximately 12–15 minutes to complete. It covers system conditions, your perceptions of how reliably your system responds, and your day-to-day operational practices. You will receive a small incentive of airtime for your participation.
Your responses are voluntary and confidential. Identifying information will be stored separately from your survey responses, and findings will be reported in aggregate only. No individual respondent will be identifiable in any publication or presentation.
