Home-based records

A home-based vaccination record is a medical document issued by a health authority on which an individual’s history of vaccinations received from all healthcare providers is recorded. In contrast to a facility-based record, the home-based record is maintained in the household by an individual or their caregiver and brought to the health clinic/post to be completed by a health worker at each time of vaccination.

To support the exchange of information related to the design and content of home-based records, this website provides a repository of home-based records that have been used in many countries around the world. If you are looking for home-based records from a particular country, click the country in the world map below, or select the country in dropdown box under the world map, to view all home-based records from that country. Alternatively, search for home-based records with particular characteristics, such as language, size, format, etc. A list of additional resources on home-based records and their use is also available.
